Two things, this reflects the potential for hegemony by the Large States or in this case the Western States over the rest of the country. Seattle, Portland and particularly California have peculiar geographical and topographical challenges to overcome regarding pollution, transportation, economics etc.

While they like to grand stand and get credit for being "progressive" on things like environmental policy - what they are really doing is trying to get the rest of the country to play by their rules so that they can we will end up paying for their way of life ans they can spend their own money on themselves.

AS for privacy, there was an article not long ago detailing how a researcher figured out how every member of a community voted on each ballot issue by comparing several disparate and supposedly private government databases(eligible voters, actual voters, numbers on ballots, numbers on voting cards, voting tallies, time stamp on ballot etc.); and I suspect that in the medical field with a little homework, accessing public records any doctor, pharmacist, drug company, insurance company and employer and government official could figure out every medicine you take and where you get them filled; although again all the information is not in one database and is supposedly private.
